How Much Will Tubal Reversal Surgery Cost?

Tubal Reversal Fee Structure

Initial Consultation Fee: $225

This fee includes a patient's initial consultation with Dr. Storment either in-office or by phone. An initial consultation is required to determine whether tubal ligation reversal surgery is right for you. Dr. Storment must have your operative reports from your tubal ligation surgery to conduct this consultation. If you do not have access to your operative report you can opt for the laparoscopy option below.

Tubal Reversal Surgery Fees:

You can schedule your surgery appointment and hold a surgery date with a $250 down payment. The balance of the surgery fee is due 30 days prior to the surgery date. Your payment can be made by cashier's check, money order, VISA, MasterCard, or by personal check (must clear 14 days before scheduled surgery date). All fees are all-inclusive and cover the doctor’s professional fee, the anesthesiologist's fee, supplies, operating room charges and facility fees, pre-operative consultation with your surgeon and follow-up by phone and e-mail.

The fee for Tubal Ligation Reversal surgery includes:

  • Pre-operative consultation appointment the day prior to surgery
  • Surgeon's fee
  • Anesthesia fees
  • Surgical supplies
  • Operating facility fees
  • Follow-up care

*NOTE: With Tubal Ligation Reversal Surgery with Laparoscopy Option, if during laparoscopy it is determined that your tubes cannot be reversed, you will be refunded $3000.

All fees must be paid within 30 days of surgery or surgery date will be cancelled. Please see Cancellation Policy below.

OTHER Potential costs:

  • Semen Analysis $150
  • Egg quality testing $200
  • HSG $500 (most patients don’t require this)

Medical Insurance and Tubal Reversal Surgery

Some insurance companies may cover part of the cost of tubal ligation reversal surgery. However, we do not accept payments from insurance carriers for tubal reversal surgery. If your insurance carrier will cover part or all of the cost of your surgery, please be sure they send the reimbursement check to you directly. Our finance coordinator will be happy to assist you with filling out any paperwork required for a claim to your insurance company. However, it is the patient's responsibility to submit the claim.

Any pre-certification or pre-authorization that may be required by insurance policies are the responsibility of the patient. Since our standard and discount fees are all-inclusive, we do not provide a cost breakdown to insurance companies.

Cancellation Policy

If you cancel your procedure for any reason within thirty (30) days of your scheduled surgery date, there will be a 25% cancellation charge based on the total amount of your surgery. A $1,000 rescheduling fee will apply within this time frame. Rescheduling or cancelling of your surgery within seven (7) days of the surgery date, regardless of the reason, will result in forfeiting 100% of the surgical fee. Refunds on cancelled procedures may take up to 30 days.
